Meeting Patients Where They Are

Edwin serves as the Manager for Revere Medical's ACO Medical Ambulatory Care Management team. In his role overseeing the Medicare program, Edwin and his team take on the vital task of helping vulnerable populations manage their health while navigating an incredibly complex healthcare system. When asked what he loves most about his day-to-day work, Edwin emphasized the profound impact of keeping patients where they want to be most: at home.

"The most rewarding part of my role is knowing that my team is making a meaningful difference for our Medicare population. We help patients address not only their medical needs but also navigate an often-complex health care system. Supporting them in this way helps them remain healthy, safe, and independent at home."

The Power of Teamwork and Persistence

Care management rarely comes with a one-size-fits-all solution. Real impact requires patience, strategy, and cross-functional collaboration. Edwin recently celebrated a major win that perfectly illustrates this ethos. His team was working with a specific member who had a history of high hospital readmissions and had proven difficult to consistently engage. Rather than giving up, Edwin’s interdisciplinary team came together to build a personalized approach. By coordinating communication and care between the RN Care Manager, the primary provider, visiting nurse services, and elder services, the team successfully engaged the member. The patient accepted several essential resources, which ultimately drove down their hospital readmission risk.

"It was a great example of how teamwork and persistence can lead to better support for a patient," Edwin notes.

Recharging on the Court

While Edwin spends his working hours navigating complex medical webs for his patients, he prefers to spend his free time navigating the baseline of a tennis court.

‍

"I enjoy playing as much tennis as I can," Edwin says. "I appreciate that it is both physically active and mentally engaging, and it is a great way for me to recharge and decompress."
